Public health, community health, and population health are interconnected approaches with distinct areas of focus that often overlap in practice. The descriptions below highlight the primary characteristics of each approach.

SBRHN's approach to Community Health​​​

Identify and Address Health Disparities
Recognize differences in health outcomes across different groups or towns or neighborhoods, especially those impacted by social, economic, or environmental factors. Focus efforts on populations facing higher risks.

Promoting Healthy Behaviors
Encourage behaviors that improve overall health, such as physical activity, balanced nutrition, tobacco cessation, recovery supports and preventive care, tailored to community needs and culture.

Improving Access to Healthcare
Work to remove barriers—financial, geographic, linguistic, or systemic—that prevent people from getting timely, quality health services.

Advocating for Policies and Programs
Support legislation, funding, and community programs that enhance public health infrastructure, address social determinants, and create healthier environments.

Collaborating with Community Partners
Build partnerships with local organizations, healthcare providers, schools, businesses, and residents to leverage resources and create sustainable health improvements.
