Support for Students - Support for Families

Who We Are

SBCCC is a community program designed to deliver coordinated, cross-system support services for at-risk students and their families.​

SBCCC helps students navigate community resources and services more effectively, fostering increased support, stability, and overall health and well- being.

What We Do

Develop a care team that centers the student and their family/caregiver. 

The student and their parent or guardian choose who will participate on their care team. This could include:

​medical providers, teachers or other school staff- like a nurse or a counselor​, a therapists or other behavioral health clinician & community service providers, an advocate, or mentor!

How We Help

The care team for each student centers their needs and priorities. We create a personalized care plan and help students and their parent/guardian navigate services and access resources.

Coordinated care makes it easier to increase support across school, health, and community systems & build a strong network of care tailored to the student and their needs.
