Housing
Affordable Housing
Primary Partner: The Great Barrington Affordable Housing Trust Fund

This new affordable housing project on Park Street in Housatonic is in the early stages of development. The Southern Berkshire Rural Health Network (SBRHN) is serving as a convener, bringing the community together around this important initiative.
SBRHN is excited to partner with the Great Barrington Affordable Housing Trust Fund to support their critical work, whose mission is to provide housing options that are affordable to low and moderate income households in Great Barrington.
As part of this partnership, SBRHN will lead efforts to gather meaningful community input through inclusive, accessible, and effective outreach including community meeting facilitation and user-friendly surveys.​
Transportation
Micro Transit Program
Primary Partner: South County Connector- Town of Great Barrington

The South County Connector Micro Transit Program is a vital resource that enhances mobility for some of the most vulnerable members of our community. The program prioritizes the transportation needs of older adults, individuals with disabilities, and those requiring access to healthcare and employment opportunities.
The SBRHN has been a strong advocate for the feasibility, expansion, and long-term sustainability of the South County Connector Micro Transit Program. Our support includes community and civic engagement, securing research and studies, and obtaining funding to expand ridership capacity.
We also actively collaborate with micro transit leaders across the state to learn and share knowledge and best practices. SBRHN remains deeply committed to this long-term partnership and to supporting stability and future growth of the Micro Transit Program.

Youth
Pediatric Care Coordination
Primary Partners: Five
school districts and and two Medical Providers

The Southern Berkshire Rural Health Network (SBRHN) has been a strong advocate for the pediatric care coordination model within our community. This effort has included securing additional funding to support and expand the Southern Berkshire Community Care Coordination (SBCCC) program.
During the 2025–2026 school year, the program will grow to include five school districts in South County: Southern Berkshire (SBRSD), Berkshire Hills (BHRSD), Lee Public Schools, Lenox Public Schools, and Farmington River.
Community Health Programs (CHP) and Macony Pediatrics will also be key partners in this expansion. SBRHN will oversee funding and reporting, provide cross-system program development and operational support, and facilitate training and professional development opportunities.
SBRHN Strategic Planning
The South Berkshire Rural Health Network (SBRHN) is excited to launch a strategic planning process aimed at strengthening and defining the rural health network. Rooted in our mission to empower health and wellbeing – building vibrant communities, and inspired by our vision to foster a culture of health in South Berkshire County, this plan will help us deepen our impact, prioritizing equity and access for all who live, work, and play in South Berkshire County.
