Community Health Needs Assessment (CHNA)

The Community Health Needs Assessment (CHNA) is a collaborative process that identifies the most pressing health needs within our county. The primary goal is to improve health outcomes by guiding hospitals and public health systems in prioritizing and addressing local health concerns.

Based on the findings of the CHNA Hospitals develop Health Improvement Plans that target identified priorities. Local leaders and residents play a key role by serving on a Regional Advisory Committee (RAC).

​

A core value of the CHNA is health equity, ensuring that all individuals have a fair and just opportunity to achieve their highest level of health.

Berkshire County Community Health Improvement Plan (CHIP) 

The data from the CHNA informs our County Health Improvement Plan (CHIP) and other local community planning efforts.

 

The CHIP implements strategies to improve the overall health and well-being of Berkshire County residents.

 

The current plan focuses on addressing social determinants of health, promoting equitable access to healthcare, preventing chronic diseases, and enhancing mental health services through coordinated community efforts and data-driven interventions.

The County Health Initiative
(CHI) 

The Berkshire County Health Initiative (CHI) is a collaborative effort aimed at improving the health and well-being of residents across Berkshire County.

 

We partner with the goal to improve the overall well-being of all people in Berkshire County by taking a holistic approach to building a more healthy, vibrant, and inclusive community.

 

The CHI Advisory Committee is a group of decision-makers from a variety of disciplines that are committed to the mission of the CHI.
